Are there any good websites to create an online form for free, by an order of image?

I have a small photography business and I recently moved location, but needs a good command line to allow my clients to the house so that I shot while so that we can not wait for submit! Wufoo and I tried some others, but none seems to give me the options you need to allow the client to compose your image number, size and the quantity of each size! Please help!
